Our referral process is very simple. To submit an application, please complete the ‘New Participant Enquiry Form‘ located below.

Once we have received your application, a member of our friendly Client Services team will be in touch to find out more about the support needs and goals of the participant, as well as answer any questions you may have regarding the NDIS, payment and fees.

Following this, our Client Services team will arrange a site visit with the participant to conduct an assessment.
